The Alzheimer’s Planning Center at Carolina Family Estate Planning is looking for a Care Manager to help clients and their families make plans for what kind of care they’ll need, where they should get it, and how to get it in the best way possible.

This job is primarily about making connections with people. The right candidate is upbeat, cheerful, warm, helpful, and friendly. They naturally build relationships quickly, informally, and comfortably, and easily gain the enthusiasm and cooperation of others (bringing together a husband or wife and adult children with different points of view on how Mom or Dad should be cared for). You’ll need to be able to read other people, listen to them, and empathize with them.

Once you’ve learned what your clients and their family members want and need, you’ll work with the rest of our team (attorneys, paralegals, financial advisors, and relationship managers) to come up with a list of recommendations. Your role is to be the expert on your clients’ quality of life and to coordinate with our legal and financial experts to put in place a plan that works. And then stay in touch with your clients to help them have the best life possible for their remaining years.

You’ll need to be organized and flexible: There will be a fair degree of variety from one day to the next, especially with regard to personal interactions. There will be several "pots on the fire" at once, and a relatively quick pace is required to keep everything going smoothly. This position will be hybrid in nature. The schedule of working in the office and out will be determined upon the completion of training.

• This is a new role at the firm. You will be working with the firm’s Leadership Team to develop systems, based on successful implementations at other firms, to deliver the services you will be provided soon after onboarding
• With the firm’s attorneys and relationship managers, explaining the firm’s process for delivering services to clients at Strategy Sessions
• Conducting home interviews and evaluations at clients’ homes to understand their current living situation and develop a care plan to improve their current and future circumstances
• Following up with clients to check in, gather information, and schedule appointments
• Community outreach and marketing: Working closely with marketing staff to assist with the promotion of the firm, including but not limited to, visiting senior care communities and professionals, presenting seminars, writing articles, radio speaking engagements, speaking at events, and relationship building with existing and potential referral sources
• Researching community resources to ensure all firm information of facilities, resources, and materials are up to date to assist clients engaged in special needs or dementia planning or guardianships

• Bachelor’s degree in health care, related social work or human services, or another related field. 2-4 years of experience in an approved field of work. Prior experience in the field of dementia care or working with the elderly population is preferred
• Strong interpersonal, customer service, and conflict management skills. Develops and maintains friendly and productive relationships with others. Responds promptly to client needs, solicits client feedback to improve service, responds to requests for service and assistance, and meets commitments. Do not shy away from crucial, sometimes challenging, conversation topics
• Strong oral and written communication. Do not hesitate to get on the phone with a client or their family to explain or work out issues. Documents what communication has occurred in the firm’s Case Management System. Follow up with clients and family with a written summary
• Organized and punctual. Maintains a calendar of client and internal appointments with adequate travel and preparation time. Begins and ends appointments on time
