We are looking for an experienced Admissions On-Call Nurse to join our team in Lancaster, OH. The ideal candidate will be a professional, organized, and dependable individual with a current valid nursing license and a minimum of two years of experience in a medical or healthcare setting. This individual will serve as a FAIRHOPE representative for admitting new patients and be responsible for obtaining the necessary clinical information for admission, recording complete and accurate patient assessment and history information, and providing patient education and discharge planning. The nurse will also ensure that patient care meets quality and safety standards, coordinate the transfer of information to other healthcare providers, and provide support to other staff members. To qualify, the candidate must have a valid nursing license and a minimum of two years of medical or healthcare experience.

• Facilitate FAIRHOPE admissions for new patients by assessing patients’ needs and developing a plan of care.
• Triage calls and coordinate all patient care delivery after hours and on weekends.
• Initiate and apply appropriate preventative, therapeutic, and rehabilitative nursing procedures and techniques.
• Assist patient/caregiver/family in working with other health care personnel in providing continuity of care.
• Electronically document any direct services provided and ongoing communication with patients, families, and community providers.
• Remain HIPAA compliant at all times.

• Candidates must be a graduate of an accredited school of professional nursing. Must have a current nursing license from the Ohio Board of Nursing.
• Current CPR certification.
• The ability to analyze and implement proper action is essential.
• Computer skills are required.
• Experience working with end-of-life care and/or healthcare setting preferred.
